Professional teeth whitening uses concentrated peroxides to eliminate stains and tooth discoloration. These whitening agents penetrate the porous structure of enamel and decompose pigment molecules responsible for discoloration. Unlike over-the-counter whitening products that use low concentrations, professional whitening uses hydrogen or carbamide peroxides at concentrations of 25-40%, which produces much more dramatic results. There are two types of discoloration: extrinsic (surface) and intrinsic (internal). Extrinsic discoloration is caused by external factors like smoking, coffee, tea, red wine, or colored foods consumption. It is generally easier to treat. Intrinsic discoloration results from internal factors like natural aging, exposure to certain medications during tooth formation (like tetracycline), or dental trauma. Intrinsic discoloration is more difficult to treat but professional whitening can significantly improve appearance even for these cases.
Professional in-office whitening is generally fast (30-60 minutes) and offers immediate results. We first apply a protectant on your gums to protect them from whitening gel. Whitening gel is then applied to tooth surface. An LED or UV activation light may be used to accelerate the process. The gel is left in place for 15-20 minutes, then rinsed. This process can be repeated several times in one session depending on desired whitening level.
Professional whitening can lighten teeth by several shades in one session. Many patients see improvement of 6-10 shades on the Vita scale. Results can last 3 to 6 months with normal eating habits, or longer if you avoid colored foods and drinks. Smokers generally see results diminish faster (1-3 months) due to continued nicotine exposure.
To prolong results or for patients who prefer gradual whitening, we offer home whitening kits. These kits use lower concentrations (10-16%) applied via custom-made trays fabricated at the laboratory. The patient applies the gel at home for 30 minutes to several hours daily for 1-2 weeks. Although slower, this method is more comfortable and results are equally effective as in-office whitening.
Some patients experience temporary sensitivity after whitening, particularly if their teeth were already sensitive. This sensitivity is usually mild and disappears after a few days. To minimize sensitivity, we use a desensitizing gel applied before whitening. We also recommend using desensitizing toothpaste after whitening.
Whitening is not effective on existing restorations like crowns, bridges, or veneers. These restorations don't change color with whitening, which can create a shade mismatch. If you have visible restorations and are considering whitening, we recommend replacing restorations after whitening to match the new shade. Whitening is not recommended in children under 14 years because their dental nerves are still sensitive.
To prolong your results, avoid or limit colored foods and drinks (coffee, tea, red wine, tomato sauce) for the first 48 hours after whitening. Maintain excellent oral hygiene: brush twice daily and floss daily. Avoid smoking. To prolong results indefinitely, you can do touch-ups every 6-12 months, either in office with a quick session, or at home with your personal kit.
